Don’t forget the mandatory requirements for ADSE Requests:
- Clinic/Doctor’s Letterhead
- Full name of the patient
- ADSE beneficiary number
- Medical Act designation according to the ADSE Table
- Doctor’s Label and Signature
- Patient’s Signature, on the front of the request
- 1 test = 1 request, in case of simultaneous tests
- Requests cannot have corrections, erasures, or other modifications
